Second account

What a direct deposit authorization form does

It is the written permission your employer's payroll team needs before sending your pay electronically to your bank. It names you, names the employer, lists the account or accounts that should receive the money, and records the date you agreed to it.

What you need before you start

Your bank's name, the nine-digit routing number and your account number, and whether the account is checking or savings. A check shows the routing number at the bottom left; your bank's app or a bank-issued account letter shows both numbers if you do not have checks.

Splitting pay across two accounts

Many employers allow a split deposit. Add a second account, then decide what the first account receives — a fixed dollar amount or a percentage of net pay. The second account receives whatever is left, so the whole paycheck always has a destination.

Routing and account numbers

The form checks that a routing number is nine digits long, which catches most typing mistakes. It does not contact your bank and cannot confirm that an account exists, so read both numbers back against your bank's own records before you hand the form in.

Why the preview hides your numbers

On screen, all but the last four digits of each routing and account number are hidden, so a shared or public screen does not display your bank details. The downloaded and printed form shows the complete numbers, because that is what payroll needs to set up the deposit.

Signatures and effective dates

Typing your name records who authorized the deposit and when. Some employers require a handwritten signature or their own form; if yours does, print this one and sign it by hand, or use theirs. Give payroll enough notice that the change can take effect before the next pay date.

Changing or cancelling direct deposit

A direct deposit authorization normally stays in place until you change or cancel it in writing. If you switch banks, submit a new authorization and keep the old account open until you see a deposit land in the new one.

Keeping a copy

Because this tool stores nothing, the PDF you download is your only copy. Save it somewhere private — it contains full bank numbers — and share it only with your employer's payroll contact.

Do I need an account?

No. There is no signup, no login and no email collection. Open the page, fill in the form and download the PDF.

Does PaycheckVista see my bank details?

No. The form is assembled in your browser and the PDF is produced there too. Your name, your employer's details and your routing and account numbers are never uploaded, and nothing is saved between visits.

Why are my account numbers hidden in the preview?

So a screen someone else can see does not display your full bank details. The preview shows only the last four digits. The downloaded and printed form contains the complete routing and account numbers.

Can I split my paycheck between two accounts?

Yes. Add a second account and choose whether the first receives a fixed dollar amount or a percentage of net pay. The second account receives the remaining amount.

Is the routing number verified?

No. The form checks only that it is nine digits, which is the standard length for a U.S. routing number. PaycheckVista does not contact any bank and cannot confirm that an account exists.

Does typing my name count as a signature?

On this template, the name you type is printed on the signature line as your authorization, the same as every other field you fill in yourself. Some employers require a handwritten signature — print the form and sign it by hand if yours does.

Will my employer accept this form?

Many employers accept a general authorization, but plenty use their own form or an online payroll portal instead. Ask your payroll contact what they require before relying on this one.

Do I need to attach a voided check?

Some employers ask for a voided check or a bank-issued account confirmation alongside the authorization. The form includes a line reminding you to attach one if that applies.
